HUG Documentation Checklist
The following documents must be submitted with your application. If you have any questions or need assistance, please call (404) 362-1200. Improper documentation will cause delays and your application may be cancelled. You will have 5 DAYS from the date of your appointment to return any missing information.
- Social Security Card, Driver's License or Other ID of each Homeowner & Household Members age 18 & up.
- Warranty Deed or Quit Claim Deed
- Property Taxes Verification of payment & Current Assessment
- Homeowner's Insurance Verification of premium paid & current policy
- Current Mortgage Statement
The following information is required for both the applicant and ALL Household Members over 18 years old.
- Last three check stubs, name, address and phone number of Employe(s).
- Most recent award letter or regular benefit check(s); pension, disability, social security, and SSI.
- Other Source of Income
- Applicable Asset Statement(s) - Most Recent Statement
- Checking/Savings Accounts
- CD's
- IRA, etc..
- Whole Life Insurance
- Other

You should complete the HUG form and bring it with you to your appointment.
